Printable workflow checklist
Give every recorded hour a clear history.
An organized log connects the activity to its setting, training stage, documentation, and review.
- Activity date or clearly defined reporting period, actual duration, and category.
- Internship/practicum versus postgraduate/associate track.
- Workplace or placement, role, and effective employment/placement dates.
- Responsible supervisor, discipline, credential, and qualification documentation.
- Supervision session date, duration, format, and applicable relationship or agreement.
- Underlying documentation completion and required co-signature status.
- Date entered, submitted, reviewed, signed, and corrected.
- Signature target and its source: board, school, agreement, or internal workflow.
- Original signed version and any later amendment.
- Copies of required reports and submission confirmations.
A reminder is not a board rule.
Label the source of each deadline. A practice workflow target can help keep records current, but should not be presented as a universal requirement.
Review across all workplaces.
Separate sites and concurrent jobs clearly. Check for duplicated daily entries and weekly totals. Preserve signed records and document corrections without silently replacing the original.
Keep client details out of general tracking tools.
Activity categories and aggregate time often support organizational tracking without copying clinical notes. Follow your organization’s approved process for any identifying information.
